Getting to Know You
Before we begin treatment, it’s important for us to know about your medical history. This helps us know what may or may not be helpful to you and what we really shouldn’t do. Some medications make you more prone to orthopedic pain or injury, so knowing what you’re taking is also critical. Our forms also ask you to mark a drawing with the location of your pain, so that we know exactly where you are having problems before we even start. General permissions and insurance info are those forms you always have to fill out at medical appointments. We have to follow the rules and have these done before treatment.
